I hereby give my consent to the processing of my personal data for the purposes of the recruitment process carried out by the Administrator, activities aiming at my employment in the Administrator’s company and at a possible further stage of employment.

The Administrator and recipient of my data is:

ALVO Medical Sp. z o.o. with its registered office in Śmigiel (64-030), at Południowa 21A Str.

This consent covers the processing of the following categories of my data: name, surname, date of birth, passport or identity card number, PESEL (ID) number, residential address, telephone number, e-mail address.

The personal data will be processed by the Administrator for the period necessary for the current recruitment process and for the purposes of possible future recruitment for a similar position with the Administrator for a period of up to 1 year after I have provided my data, and in the event that Administrator employs me – the data will be processed for the period of this employment or any other period required by law.

I give this consent voluntarily and declare that I have been informed of my rights, i.e. the right to access, rectify, erase or restrict data processing and the right to object to processing, the right to data portability, as well as the right to lodge a complaint to the supervisory authority, i.e. the President of the Office for Personal Data Protection or any other relevant authority.

The recipients of my data, in addition to the Administrator, may be: to IT system providers, cooperating companies, and authorised entities upon documented request, for example: Courts, Attorney General, Police.

In addition, I acknowledge that providing the Administrator with the personal data indicated above is voluntary and does not constitute a legal obligation, but the failure to do so will result in my application not being considered. The provision of personal data in the scope resulting from Article 221 of the Labour Code is necessary to participate in the recruitment procedure.

I am aware that this consent may be revoked by me at any time by means of a written declaration, and that the withdrawal of consent shall not affect the lawfulness of the processing carried out on the basis of this consent prior to its withdrawal.
